AFFIDAVIT OF CHANGE OF THE NAMES LIOLAH PEWU AND LOILA PEWU TO THE NAME LOILA ALISON PEWU
1. Born unto the union of Madam Lorpu A. Pewu and Mr. Joseph F. Pewu on the 1st day of August 1983 at the Phebe Hospital, Gbarnga, Bong County, Republic of Liberia;
2. Affiant/Deponent says upon oath that from her birth, she has used and carried the name LOILA ALISON PEWU which name is reflected on most of her documents including academic ones;
3. Affiant/Deponent further says that throughout her life and school days, she has used the name Loila A. Pewu which is carried and reflected on most of her school and other documents. She also says that the names LIOLAH PEWU and LOILA PEWU were misspelt and “A” as her middle name which stands for “ALISON” was also omitted as indicated on both certificates issues by the West African Examinations Council and the Ministry of Health and Social Welfare, Republic of Liberia;
4. Affiant/Deponent further says, her actual name if LOILA ALISON PEWU instead of LIOLAH PEWU and LOILA PEWU as misspelt on both certificates. She further says that her name is spelt out, LOILA ALISON PEWU as it was misspelt inadvertently by the West African Examinations Council and the Ministry of Health and Social Welfare, Republic of Liberia, making her to request for a AFFIDAVIT CHANGE OF NAME.;
5. Deponent/Affiant therefore hereafter corrects her name from LIOLAH PEWU and LIOLA PEWU as indicated on her certificates to LOILA ALISON PEWU. However, all documents, including academic ones bearing the names, LIOLAH PEWU and LOILA PEWU remain valid to all intents and purposes.
